Required form for trip: payment and info needed. Each participants must fill out a separate form. (Parents must fill out Kid forms separately). Not sure yet about signing up? Move to step 2 and save the date for our Informational meeting. Registration for trip is March 1- May 10.
STEP 1: Registration
All mission trip participants must fill out a trip profile form. This form will include background checks, faith testimonies, and mission trip experience. Parts of It will be required for everyone over 18 years of age. (form available soon).
STEP 2: Information
Provo Informational Meeting: April 12th at 5pm. If you are interested in going on the Provo Mission trip, or would like more information before you sign up, mark your calendars to attend this informational meeting in the Sanctuary.
